CMR CONTRACT – EMPLOYER
For all who employ commercial drivers other than self-employed drivers.
CMR CONTRACT – OWNER DRIVER
For Commercial Drivers who employ themselves. If others are also employed, complete both this contract and the Employer Contract. Be sure to understand the new procedures made necessary by activation of the FMCSA Clearinghouse for Drug and Alcohol Testing Violations.
RANDOM TESTING ROSTER
For additions and deletions to the company roster of drivers. In future, this function should be available online. Presently all changes must be in writing and signed by a responsible company representative. Send the signed form to CMR by fax, email, or US mail.
COMPANY CONTACT AGREEMENT
Designate who receives reports and notifications. For self-employed drivers without an employee or co-owner who can act as a Designated Employer Representative, this Company Contact will receive mail notifications of any testing required. Within one day, when the Driver is available to test, the Contact will instruct the driver to immediately proceed to a collection site, and notify CMR.
DESIGNATED EMPLOYER REPRESENTATIVE (DER) AGREEMENT
DER must be an employee of the company with the power to remove a driver from service if there is a violation of drug and/or alcohol regulations. There should be alternate DERs so results can be reported at any time. Owner-operators may act as their own DER but should then designate a “Company Contact”, above.
